Agents of Change

I accept my appointment as a participant in the 2022-2023 school year. I recognize my responsibility to myself, my family, school and/or sponsoring agency to be the best possible participant at Agents of Change. I agree to:

1. Participate in all parts of the program to the best of my ability and be punctual for all events, including virtual events.
2. Meet as many people as possible during the session. Help others feel wanted and accepted.
3. Look for opportunities to see the positive, not the negative, in people and situations.
4. Be respectful of all students, staff, and visitors of the program.
5. Engage meaningfully in each of the weeks of programming and commit to attending every week. I will reach out to the facilitators in advance if something comes up that will not allow me to attend.
6. Refrain from outside distractions such as being on your phone or other conversations during the session.

I accept and understand the following as it pertains to my child involvement with NCCJ and the Agents of Change program:

1. I understand that this program is being offered in-person at their school or at a regional location such as a library
2. I will encourage my child to attend all programs and be entirely engaged
3. I understand that my child will engage in difficult conversations around the "isms." This includes racism, sexism, mental health, and more.
4. I consent to NCCJ contacting myself and my child through means including, but not limited to, call, email, letter.
5. I understand that by participating in this program, my child is committing to completing the program and returning to their school and community to create meaningful change.
6. My parent/ guardian and I hereby consent to having quotes of my comments, pictures and/or by NCCJ for program publicity purposes. We release NCCJ and its sponsoring agencies from any obligations or liability arising from use of said comments and pictures.
